Evaluation Checklist

The St. Jude LIFE study team looks forward to your visit. To help it go as smoothly as possible, we have created the following checklist and reminders for you.

Before Your Visit

• Confirm your visit dates. The St. Jude LIFE team will send an e-mail  or text with your visit dates.
• Complete your St. Jude LIFE surveys. About 8 weeks (about 2 months) before your visit, you will get a packet by mail. It will include your St. Jude LIFE surveys.  Please give yourself about 2–3 hours to finish them. You may skip any questions you do not want to answer. But your answers are important to health care providers and researchers.
• Mail surveys back in the stamped return envelope as soon as possible. Please send them back a few weeks before your visit. This information helps the St. Jude LIFE team prepare for your visit.
• If traveling by plane or train, call the St. Jude Travel Office (901-595-5000) at least 15 business days before your visit. The Travel Office will make your plane and hotel reservations.
• If traveling by car and you need a place to stay in Memphis, call the St. Jude Patient Services office (901-595-4501). They will make housing reservations for you either in on-campus housing or a local hotel.
• For those traveling by car, locate available parking areas on the St. Jude parking map.
• Sign up for the MyChart app if you do not have it. You can view your visit schedule and see test results.
• Check your schedule to see when your lab tests are. They may be on your first day. These are fasting labs. This means you cannot eat anything after 12 a.m. (midnight) on the day of your test. You can drink water and take needed prescription medicines.
• Make sure you know your Social Security Number (SSN) or Taxpayer ID. You need this to fill out a W-9 form for taxes. If you do not have an SSN or Taxpayer ID, let the staff know as soon as possible.
• If you find out you cannot attend your appointments, please let the St. Jude LIFE staff know as soon as possible. This allows them to reschedule that time.

During Your Visit

• Wear comfortable clothes and athletic shoes.
• Follow all instructions to prepare for tests and special procedures such as colonoscopies and fertility tests. (Not all participants have these on their schedule.)
• Check in at the registration desk each day. Please arrive 30 minutes before your first appointment. The registration staff can print a copy of your schedule if you need one. Your schedule is also in your MyChart app.
• Check in at each location where you have a test or procedure scheduled. That helps hospital staff know you have arrived for your appointment.
• If you cannot attend an appointment, please let the St. Jude LIFE staff know as soon as possible.
• All participants and other children under 18 must have adult supervision. Child care will not be provided.

After Your Visit

• Check your MyChart app for results. You may get some results during your visit.
• Check your mail for your incentive check. You should get it 6–8 weeks after your visit.
• Call the St. Jude LIFE staff if you have any questions or concerns.
